Please register the players you have at the time of registration. - If you have more than 12 players, list your first 12 players on this form and send the additional names to adrouin@fondationdespompiers.ca
For any subsequent player changes, please submit them at least two days before the start of the tournament to adrouin@fondationdespompiers.ca


You must provide proof of employment/eligibility by uploading the documents using the Browse button on the form.
If you do not have proof for all players at the time of registration, please send us the documents you already have and submit the remaining proofs before the registration deadline (otherwise, your registration will be rejected) to adrouin@fondationdespompiers.ca
